Description

  • Regularly engage with patients and track adherence to their care plans.
  • Monitor patient device readings and investigate out-of-bound values.
  • Maintain accurate records of patient interactions and program details.
  • Stay current on regulations and compliance standards related to CCM/RPM services.
  • Ensure patient enrollments comply with legal and ethical standards.
  • Track care performance and work to meet or exceed goals.
  • Use technology to document patient information and communicate with patients and the team.
  • Support patients throughout their healthcare journey with compassionate care.

Requirements

  • National certification in Medical Assisting from AAMA, NHA, NCCT, NAHP, or a similar organization.
  • Single-state certifications, diplomas, or completion certificates do not qualify.
  • Must reside in the United States.
  • Strong written and verbal communication skills.
  • Empathy and the ability to connect with patients and their families.
  • Basic knowledge of chronic health conditions and their management.
  • Comfortable using technology for documentation and communication.
  • Attention to detail and strong organizational skills.
  • Ability to work independently and as part of a team.
  • Please attach your Medical Assistant certificate when applying.
