Description

  • Support an assigned panel of patients with care navigation, appointment logistics, prescription support, lab support, referral coordination, and care plan adherence.
  • Conduct virtual and in-person outreach, including initial intake, outbound calls, and appointment follow-up.
  • Assess social determinants of health needs and connect patients to community resources and SDOH programs.
  • Collaborate with a multidisciplinary care team to contribute to care plans, triage requests, and resolve complex patient needs.
  • Document patient and care team interactions across multiple systems and tools.
  • Provide culturally and linguistically appropriate health education and information.
  • Support individualized goal setting using motivational interviewing and conduct social needs assessments.
  • Assist with patient registration and enrollment, including community engagement activities and relationship-building.
  • Participate in community events, facilitate group education sessions, and represent Zócalo Health in the community.
  • Coordinate internal clinical services and support continuity of care through scheduling, referrals, and program enrollment.

Requirements

  • Must reside in Inyo, Mono, or Alpine County and be able to work remotely with some in-community responsibilities.
  • Must be authorized to work in the United States.
  • Fluency in both English and Spanish, spoken and written.
  • High school diploma or GED required.
  • California CHW certification, or 2,000+ hours of CHW work in the past three years with willingness to obtain certification within 18 months of hire.
  • 1-3 years of healthcare experience or healthcare navigation experience in the community.
  • 2-5 years of community work, advocacy, engagement, or organizing experience.
  • Previous experience in health promotion, project coordination, social research, or administration.
  • Familiarity with Google Workspace preferred.
  • Experience documenting in an EHR preferred.
  • Training in motivational interviewing preferred.
  • Comfort with multiple computer applications and willingness to learn new technologies and frameworks.
  • Strong ties to the local Latino community and knowledge of local resources for SDOH needs preferred.
  • Willingness to travel to other communities and have reliable transportation.
  • COVID-19 vaccination or approved accommodation, with at least one shot before the first day of employment.
