Description

  • Guide members through the healthcare system and help them access appropriate, high-quality, cost-effective care.
  • Help members find providers, understand referral options, and coordinate healthcare services.
  • Communicate with members, providers, and external partners to gather information, resolve routine issues, and facilitate care.
  • Collaborate with Care Navigators, Sana Care providers, Medical Assistants, and Customer Support to deliver a seamless member experience.
  • Maintain accurate and timely documentation of member interactions and care-navigation activities.
  • Manage tasks through completion using established workflows and escalate complex clinical, benefits, or operational questions.
  • Identify workflow improvement opportunities and share feedback with the team.
  • Provide compassionate support that helps members feel heard and confident in their healthcare journey.

Requirements

  • 2+ years of experience in a fast-paced environment, including at least 1 year in care navigation, referrals, provider scheduling, or healthcare benefits.
  • Strong written and verbal communication skills with an empathetic, professional approach.
  • Excellent organization, attention to detail, and ability to manage multiple tasks reliably.
  • Comfort learning new technology and navigating multiple systems in a fast-paced remote environment.
  • Resourcefulness, proactive problem-solving ability, and sound judgment about when to escalate issues.
  • Collaborative approach, openness to feedback, and commitment to helping teammates succeed.
  • Adaptability and willingness to learn and take on evolving team needs.
  • Passion for making healthcare more accessible, affordable, and easier to navigate.
